Medicare Enrollment Guide

Who Qualifies?

This is for people who are first becoming eligible for Medicare, most often around age 65, and for some people who qualify earlier because of disability, ESRD, or ALS.

Enrollment Timeline

Your IEP is a 7-month window that begins 3 months before the month you turn 65, includes your birthday month, and ends 3 months after. Missing this window can create coverage delays and possible Part B or Part D late-enrollment penalties.

Key Considerations

Review Original Medicare Part A and Part B, Medicare Advantage Part C, standalone Part D, and Medigap options. Doctors, prescriptions, pharmacies, travel needs, premiums, and expected out-of-pocket costs should all be checked before you enroll.

Who Should Review Coverage?

AEP is for people already on Medicare who want to review or change Medicare Advantage or Part D coverage for the next plan year.

Enrollment Window

AEP runs October 15 through December 7 each year. Changes generally take effect January 1. This is also when many people review their Annual Notice of Change, also called an ANOC, to see what is changing for the coming year.

Available Plan Changes

During AEP, you may be able to switch Medicare Advantage plans, change Part D plans, move from Original Medicare to Medicare Advantage, or return from Medicare Advantage to Original Medicare and consider separate drug coverage.

Qualifying Events

A SEP may be available when certain life or coverage changes happen, such as moving, losing employer or union coverage, entering or leaving a plan service area, qualifying for Medicaid, or receiving Extra Help with drug costs.

SEP Eligibility

SEP timing depends on the event and the coverage you already have. Some windows are short, so it is best to review your options as soon as the change happens.

Coverage Options

A SEP may allow you to enroll, disenroll, or switch plans outside AEP. The choices available depend on the qualifying event, your current coverage, and the type of Medicare plan involved.

Coverage Overview

Medicare Advantage, standalone Part D, and Medigap serve different needs. Medicare Advantage is private Medicare coverage, Part D helps with prescription drugs, and Medigap works with Original Medicare to help reduce certain out-of-pocket costs.

How Each Plan Differs

Medicare Advantage plans often use HMO or PPO networks and may include extra benefits. Medigap usually offers more provider flexibility with Original Medicare, but generally does not include Part D drug coverage. Part D plans vary by formulary, drug tier, pharmacy, and prior authorization rules.

Compare Before Enrolling

Compare more than the premium. Review MOOP limits, copays, coinsurance, formularies, drug tiers, preferred pharmacies, referrals, prior authorizations, provider networks, and whether underwriting may apply for Medigap outside certain windows.

Yes. California Benefits Insurance Center can review doctors, medical groups, hospitals, prescriptions, drug tiers, pharmacies, and formularies before you apply. This is especially important because Medicare Advantage networks and Part D formularies can change from year to year.

Medicare Advantage is private Medicare coverage that may use a network and often includes extra benefits. Medigap works with Original Medicare and can reduce certain out-of-pocket costs, but it usually does not include Part D prescription drug coverage. California Benefits Insurance Center can help explain how each option fits your needs.

No. Sometimes the best choice is to keep what you already have. California Benefits Insurance Center compares your current plan, doctors, prescriptions, costs, and benefits against available options so you can make a confident decision.

You may have to wait for another enrollment window unless you qualify for a SEP. Depending on the situation, missing the right window can also create coverage gaps or late-enrollment penalties. California Benefits Insurance Center can help you understand which enrollment options may be available.

Medicare Advantage Plans (Part C)

Medicare Advantage Plans—often called MA Plans or Part C—are offered by private, Medicare-approved insurance companies. These plans combine the benefits of Original Medicare with additional features and services.

Types of Medicare Advantage Plans include:

  • Preferred Provider Organizations (PPO)

  • Health Maintenance Organizations (HMO)

  • Private Fee-for-Service Plans (PFFS)

  • Special Needs Plans (SNP)

  • Medicare Medical Savings Account Plans (MSA)

When you enroll in a Medicare Advantage Plan, your Medicare-covered services are provided through that plan rather than through Original Medicare. Many of these plans also include prescription drug coverage and additional benefits such as dental, vision, or hearing care.

Because Medicare Advantage Plans are county-specific, your available options will depend on where you live in California. Comparing plans can help you find the right balance of coverage, flexibility, and cost for your healthcare needs.

Medicare Supplement Insurance (Medigap)

A Medicare Supplement Plan, also known as Medigap, works alongside Original Medicare to help pay for out-of-pocket costs such as deductibles, copayments, and coinsurance. These plans are offered by private insurance companies and are designed to fill the “gaps” that Medicare does not cover.

Key features of Medigap plans include:

  • Freedom to visit any doctor or specialist who accepts Medicare.
  • Coverage that travels with you throughout the United States.
  • Standardized plan options (A through N) with predictable costs.

Unlike Medicare Advantage, Medigap plans do not include prescription drug coverage, so you would need to enroll separately in a Part D Prescription Drug Plan if you need medication coverage.

For many seniors who prefer flexibility and predictable expenses, a Medigap plan can be an excellent choice for supplementing Original Medicare coverage.

Prescription Drug Plans (Part D)

A Medicare Prescription Drug Plan (Part D) provides coverage for prescription medications and certain vaccines that are not included under Medicare Parts A or B. These plans help reduce the cost of prescription drugs, biological products, and select medical supplies that would otherwise need to be paid out of pocket.

Even if you do not currently take any prescription medications, it is wise to consider enrolling in a Part D plan. Having coverage in place protects you from unexpected medication costs in the future and helps you avoid potential late enrollment penalties.

Eligibility and Requirements

You may be eligible for Medicare if you meet one or more of the following criteria:

  • You are 65 years of age or older.
  • You are under 65 and have a qualifying disability or End-Stage Renal Disease (ESRD).
  • You or your spouse have worked for at least ten years for an employer that contributes to Medicare.
  • You are a United States citizen or a lawfully permanent resident.

Enrollment

You can begin the Medicare enrollment process three months before your 65th birthday and complete it no later than three months after your birthday. This seven-month window is known as your Initial Enrollment Period.

You can enroll by visiting the Social Security website, contacting your local Social Security office, or calling 1-800-772-1213. Enrolling during this period ensures your benefits begin as soon as you are eligible and helps you avoid late enrollment penalties.

If you miss the Initial Enrollment Period, you can sign up during the General Enrollment Period, which takes place each year from January 1 to March 31. Some individuals may also qualify for a Special Enrollment Period if they meet certain conditions, such as losing employer coverage.

If you already receive Social Security or Railroad Retirement benefits, you may be enrolled automatically. In that case, your Medicare card will arrive in the mail about three months before your 65th birthday or after 25 months of disability benefits.
